Among the patient population in transition, there is a high rate of patient readmission. Data collected by our mental health department indicate the rate of readmission for patients with substance abuse disorders ranged from 25% to 30% and the rate of readmission for patients with depression ranged from 30% to 35%. These high rates of readmission indicate there is inefficiency in the patient transition process. This can be attributed to several factors such as lack of collaboration and coordination among health-care providers, and inadequate patient and caregiver education. Healthcare providers fail to follow-up with substance abuse disorder and mental disorder patients who are in transition. To promote lasting recovery, these patients require preparation for everyday life after discharge, and social integration into society.
